IMPORTANT: By completing this on line form you have formed a commitment to attend the selected Cardiorenal regional workshop which can only be cancelled by the deadline outlined for cancellations below.
Amendments to registration:
Any amendments must be submitted, in writing, to the meeting organisers.
Cancellations:
Places for these small evening workshops are very limited and we understand the best-laid plans can sometimes change. If you find you can no longer attend the meeting, please let us know as soon as possible so we can offer your place to someone else. Last-minute cancellations are very hard for us to fill and we will not be refunded any costs that we have spent on your behalf. The Cardiorenal Forum can only continue to offer the high-quality learning that these workshops offer free of charge to healthcare professionals, if delegates offered a free place respect this with their attendance. It is very disappointing to both speakers and other attendees if people fail to show up on the night or with little notice.
Accompanying Persons:
Guests may NOT attend the meeting or exhibition.
